Prize Winners

If you are a prize winner please read the following below:

• If you haven’t received a phone call, email or voice mail chances are we have the wrong number for you or your prize is not in yet… feel free to call the front desk and double check.
• If you won a prize and you have received a phone call – please come by our offices at the address and dates/times listed above, with a form of photo id to pick up your prize.
• You must come in and pick up your prize and you must bring a valid form of photo ID.
• Only the prize winner can pick up the prize, another individual regardless of relation or marriage cannot pick a prize up for you.
• If you cannot make a show or do not want the prize you won… please know that we will not exchange tickets or replace the prize.
• Prizes that are not date sensitive will only be waiting at the front desk for 30-business days. Anything past 30 business days will be considered a forfeit prize.

CBS Radio/102.5 KZOK cannot mail any prize of a specific value to a winner, without first obtaining proper release forms. The availability of mailing any prize is at the final discretion of the station management and promotion department. The decision will be based on value of prize, size and weight of prize, and ability to ensure safe arrival of prize.

Time sensitive material or prizes valued at and over $50 WILL NOT be mailed.

The W-9 and Prize Release Form must be filled out when claiming a prize. For your convenience, you can download them beforehand below. Release forms may also be mailed, faxed, or scanned/emailed.

• W-9 form (PDF) completely filled out, we only need PAGE 1 returned
• KZOK Prize Release Form (PDF) filled out and signed
